Definition Health is a ground-breaking, best in class digital health company that is revolutionising the hospital-based approach to healthcare. Founded in 2018 by two practising NHS Consultants, we have produced the first UK end to end digital journey for surgical patients with the primary aims; to drive improved outcomes, increase efficiency and deliver an enhanced patient experience.
We have recently been chosen by NHS England through the NHS Innovation Accelerator (NIA) as one of only twelve innovations (and the only surgical solution) to help the NHS recover from the Covid 19 pandemic. We are proud that throughout the pandemic our systems have supported the NHS and Independent Sector in the remote pre-assessment of surgical patients across multiple specialties including cancer services.
Definition Health offers;
- LifeBox – a digital pre-operative pathway provides a complete health record for patients, the Health Questionnaire is completed by the patient in the comfort of their own home and can be accessed remotely by clinical teams allowing hospitals to operate more efficiently, significantly reducing outpatient attendances and consequent pressure on pre-assessment teams. The Questionnaire increases co-morbidity capture which reduces short notice cancellations, increases patient safety and, depending on reimbursement models, allows the Hospital to optimise revenues . LifeBox also includes procedure specific videos and an inbuilt consent feature, recording the fact patients fully understand the procedure they are about to undergo, with an auditable pathway for the hospital.
- Recovery – captures and records key information about the patients rehabilitation after discharge, improved understanding of the patient experience post discharge with PROMs benefits. Recovery allows the Hospital to track patients post discharge care remotely, provides alerts if things are not progressing as they should and allows Clinicians to intervene at an earlier stage reducing readmission rates. Patients report they now feel more supported by the hospital team once they have left the hospital environment
- Secure Virtual Clinic – supports LifeBox and Recovery in providing a remote three way communication channel between the Consultant, Hospital and Patient. Patient admission packs can be digitalised, follow up information (videos, patient literature) can be shared and the patient can upload photos relating to e.g. wound recovery progress. Furthermore, the Secure Virtual Clinic also provides access to a virtual consultation facility, allowing both pre-op and follow-up consultations to be provided remotely without the need for the patient to attend hospital. Significant efficiencies can be delivered through a reduction in paperwork, hospitals having greater control over patient collateral and a further reduction in outpatient attendances.
The systems have been proven to benefit patient outcomes, increase patient throughput, reduce POA times and reduce costs for hospitals by digital transformation of paper-based processes.
